Amends AS 47.07 to require the Department of Health and Social Services to provide coordinated preventive care services to eligible medical assistance recipients in the least restrictive, home-like environment.

Required preventive care services include primary care coordination, evidence-based practice guidelines, patient education, specialty referrals, routine health assessments, outcome measurement, and other services identified by department regulation.

Authorizes the department to contract with qualified entities to provide medical home-based services and alternative care programs for the elderly.

Permits the department to provide preventive services under a federal waiver if the department finds it cost-effective, receives appropriations, and obtains federal approval under 42 U.S.C. 1396-1396p.

Requires the department to establish measures with medical, social, and economic components and provide annual reports to the legislature on services provided, participant outcomes, costs, and savings.
