Establishes a grant program in the Department of Health and Social Services to provide supplemental funding to nonprofit health care facilities offering primary care services to Medicare recipients and low-income persons.

Creates a primary health care services grant account in the general fund to be funded through legislative appropriations, federal receipts, donations, and interest, with appropriations not subject to lapse.

Directs the department to establish annual application and award procedures that prioritize funding to facilities providing the most cost-effective expansion of medical services in physician shortage areas.

Allows grants to be used to provide additional incentives to physicians to practice at funded health care facilities, subject to federal law restrictions.

Defines eligible health care facilities as nonprofit primary care clinics, community health centers, or outpatient health clinics eligible for medical assistance or Medicare reimbursement.
