Requires the Department of Fish and Game to establish a system for collecting, processing, and publishing annual statistics on days worked by commercial fishing crewmembers employed on vessels or set net sites.

Mandates crewmembers complete activity forms annually by April 15 that include their name, address, license number, employment dates, and fishery information (area and gear type).

Designates crew activity form information as confidential, except that the department may create anonymized statistical data and share information with vessel/site operators for review and with federal agencies and the Department of Labor and Workforce Development.

Allows vessel and set net site operators to review and contest crew activity form information submitted by the department, with no response presumed as non-disputed.

Sunsets the crew activity form requirement and related provisions on January 1, 2015.
