Authorizes dentists, physicians, nurses, and optometrists to use prescription forms with a checkbox in the lower right corner that, if left unchecked, allows generic substitution

Requires pharmacists to include the generic drug name and manufacturer or distributor name when dispensing brand-name prescription drugs

Exempts prescriptions filled in hospitals or facilities with established formulary systems from mandatory generic substitution requirements

Directs the Department of Health and Social Services to approve and maintain a publicly available list of prescription drugs determined to be more cost-effective generic alternatives to brand-name drugs
