Temporarily reinstates the child and adult immunization program in the Department of Health and Social Services to address gaps in federal funding since 2009.

Provides recommended vaccines at no cost to uninsured and underinsured children under 19 and adults 19 and older through eligible providers.

Requires children to receive all vaccines required for school attendance plus meningococcal, rotavirus, and human papillomavirus vaccines; adults receive influenza, pneumococcal, Tdap, and zoster vaccines.

Allows the commissioner to add additional vaccines based on federal advisory committee recommendations and available funding, or remove vaccines if diseases are declared eradicated.

Expires June 30, 2015, with intent that non-extension does not implicitly deauthorize a state immunization program; takes effect immediately.
