Creates the Sport Fishing Guide Services Board under the Department of Commerce, Community, and Economic Development with nine governor-appointed members, including licensed guides, outfitters, transporters, a fisheries board member, and two public members to license and regulate sport fishing guide services.

Establishes four biennial license types effective January 1, 2013: sport fishing guide-outfitter, sport fishing outfitter, sport fishing assistant guide, and sport fishing transporter, each with specific eligibility requirements including age, citizenship, first aid certification, and examination passage.

Requires sport fishing guide-outfitters and outfitters to maintain general liability or marine protection insurance of at least $100,000 per incident and $300,000 annually, and sets license fees with nonresident fees at least double resident fees.

Authorizes the Department of Fish and Game to collect confidential information from guides and outfitters including vessel names, fishing effort, catch data, and harvest locations; makes records available to law enforcement upon request.

Provides disciplinary sanctions for violations including license revocation, suspension, censure, probation, fines up to $5,000, and criminal penalties ranging from violations to class A misdemeanors for unlicensed guiding or transportation services.
