School districts, principals, teachers, and school personnel cannot administer questionnaires or surveys inquiring into personal or private family affairs without written permission from the student's parent or legal guardian, except as provided for the Youth Risk Behavior Survey.

School districts must provide parents or legal guardians written notice at least two weeks before administering a survey, including the administration date, content description, sponsor, contact person, review opportunity, administration method, permission instructions, and notice of the student's right to refuse.

Students may refuse to answer specific questions or decline to participate entirely in any questionnaire or survey, and parents or legal guardians may submit written denial of permission for their child's participation.

The Youth Risk Behavior Survey provided by the United States Centers for Disease Control and Prevention does not require advance written permission but requires notice to parents two weeks before administration with opportunity to deny participation.
