Adds new eligibility category (16) for adults under 65 years of age with household income not exceeding 133 percent of the federal poverty line, conditional on federal medical assistance percentage of at least 90 percent.

Expands existing eligibility groups (13) and (14) to cover persons under 19 years of age and pregnant women with household income up to 175 percent of the federal poverty line.

Requires sliding fee scale premiums and cost-sharing charges based on income for eligible disabled persons under category (12) not covered by other provisions.

Directs Department of Health and Social Services to immediately amend and submit state Medicaid plan for federal approval consistent with the Act.

Makes the state plan amendment directive effective immediately, with remaining provisions taking effect January 1, 2016.
