Requires the Department of Health and Social Services to submit annual reports by September 30 detailing enrollment, costs, savings, and expenditures for individuals eligible under the federal Medicaid expansion provision (42 U.S.C. 1396a(a)(10)(A)(i)(VIII)).

Makes Medicaid expansion eligibility conditional on the federal medical assistance percentage remaining at least 90 percent; initially allows coverage at higher federal percentages (100 percent for 2015-2016, declining to 90 percent for 2020-2021).

Requires the commissioner to immediately notify legislative leadership and the revisor of statutes before January 1, 2022, if specific triggering events occur, including: loss of Medicaid system certification, federal funding reductions, enrollment exceeding projected levels, or general fund savings falling below thresholds ranging from $8.4 million to $98.3 million cumulatively.

Directs the Department of Health and Social Services to amend the state Medicaid plan and apply for federal waivers necessary to implement the expansion, contingent on federal approval by January 1, 2022.

Makes most provisions conditional on federal approval, with implementation occurring the day after the commissioner certifies approval by the U.S. Department of Health and Human Services.
