Requires the Commissioner of Corrections to establish a reentry planning program for prisoners serving 90 or more days, with planning beginning at least 90 days before release on furlough, probation, or parole.

Mandates written reentry plans for each prisoner that include proposed residence, employment or support, treatment options, counseling services, and education or job training services.

Requires the department to establish written case plans within 90 days of sentencing for prisoners serving 30 or more days (except those sentenced to 99 years), based on risk and needs assessments.

Directs coordination with the Department of Labor and Workforce Development for post-release job training and employment assistance, and with community reentry coalitions or other reentry service providers where available.

Requires the Commissioner to assist prisoners in obtaining valid state identification cards before release, with the department paying the application fee.

Mandates annual legislative reporting by January 15 on the number of prisoners provided case plans, case plans initiated, and case plans updated in the preceding year.
