Health care insurers offering pharmacy coverage must provide coverage for diabetes treatment costs including medication, equipment, supplies, outpatient self-management training, and medical nutrition therapy prescribed by a health care provider.

Insurers cannot charge more than $100 in combined cost sharing, deductibles, and copayments for a 30-day supply of prescription insulin used to treat diabetes, except when necessary to qualify as a high deductible health plan eligible for health savings account tax deductions.

Specialty drug tier cost sharing may exceed nonpreferred brand drug costs only if insurers provide at least 90 days' notice before the terms apply.

The Act applies to insurance policies and contracts issued, delivered, or renewed on or after the effective date.
