Limits combined cost sharing, deductibles, and copayments to a maximum of $35 for a 30-day supply of prescription insulin used to treat diabetes in individual and group health insurance plans, except for high deductible health plans eligible for health savings accounts.

Requires health care insurers offering pharmacy coverage to provide coverage for diabetes treatment costs including medication, equipment, supplies, outpatient self-management training or education, and medical nutrition therapy when prescribed by a health care provider.

Allows specialty drug tier cost sharing to exceed nonpreferred brand drug costs only if insurers provide at least 90 days' notice before applying the terms, with an exception for insulin coverage under the $35 cap.

Applies to insurance policies and contracts issued, delivered, or renewed on or after the effective date of the Act.
