Allows health care providers and patients to enter into direct health care agreements for primary care services in exchange for periodic fees, separate from traditional health insurance.

Patients eligible for Medical Assistance (Medicaid) or Catastrophic Illness assistance are prohibited from entering direct health care agreements.

Health care providers must either accept new Medicare patients or maintain a practice where at least 20 percent of patients are Medicare-enrolled or uninsured.

Direct health care agreements must include written disclosures of periodic fees, service descriptions, contact information for complaints, and a prominent statement that patients are not entitled to protections under health care insurance policies.

Patients may terminate agreements within 30 days with full refund (minus services rendered), or after 30 days' notice; providers may charge termination fees up to one month's periodic fee; providers cannot discriminate based on protected characteristics or misrepresent agreements as insurance.
