Creates a new category of licensed health care entity called "complex care residential homes" under Department of Health regulation

Defines complex care residential homes as residential settings providing 24-hour multi-disciplinary care to no more than 15 individuals with mental, behavioral, medical, or disability-related needs requiring specialized services and monitoring

Distinguishes complex care residential homes from hospitals and assisted living homes in state statute

Requires the Department of Health to submit Medicaid state plan amendments or waiver applications to the federal government for implementation

Implementation is conditional on federal approval by the U.S. Department of Health and Human Services on or before July 1, 2031
