Records the death of Sam Phelps of Tuscaloosa, Alabama on April 18, 2011, at age 79.

Recognizes Phelps as a long-time attorney and civic leader who was a partner in Phelps, Jenkins, Gibson, and Fowler law firm and held memberships in the Tuscaloosa County Bar Association, American Bar Association, and Farrah Order of Jurisprudence.

Honors his service as Chairman of the Druid City Hospital (DCH) Board of Directors from 1976 to 2000, during which DCH acquired medical centers in Northport and Fayette to become one of the largest health care systems in the state.

Notes his 2005 co-founding of Bryant Bank with Paul Bryant, Jr., and Dr. Wayne May.

Extends heartfelt sympathy to his surviving family including wife Mary Ann Peak Phelps, son Scott Moore Phelps, and three grandchildren.
