Requires pharmacy benefit managers providing services to Alabama residents to obtain a valid permit from the State Board of Pharmacy before operating in the state.

Mandates biennial registration and permit renewal on or before December 31st of even-numbered years, with fees and penalties determined by board rule ($25 per overdue month for delinquent permits).

Authorizes the State Board of Pharmacy to establish disciplinary standards and impose sanctions including permit denial, revocation, suspension, censure, probation, fines up to $1,000 per offense, and operational restrictions.

Provides permit holders with due process rights including written notice of charges at least 30 days before disciplinary hearings and the right to appeal board orders to circuit court under Alabama Administrative Procedure Act provisions.

Exempts employee welfare benefit plans as defined under the Employee Retirement Income Security Act of 1974 from the registration requirements and becomes effective the first day of the third month following passage and approval.
