Extends the supplemental privilege assessment and monthly surcharge on nursing facility beds from August 31, 2013, through August 31, 2015.

Maintains the supplemental privilege assessment rate of $1,603.08 per bed annually (September 1, 2011, through August 31, 2015) with a monthly surcharge of $131.25 per licensed bed initially, reduced to $43.75 per month beginning September 20, 2012.

Requires nursing facilities to include these assessments and surcharges as allowable costs in Medicaid per diem rate calculations under Title 560 of the Alabama Administrative Code.

Permits the department to reduce assessment rates if total revenues from the assessments exceed 6 percent of total nursing facility revenues in any state fiscal year.

Terminates all privilege assessments and monthly surcharges if the Alabama Medicaid Agency enters into third-party managed care contracts with commercial managed care entities for nursing facility services, with specified exceptions for regional care organizations and PACE programs.
