HB504 Summary

• Requires the Department of Public Health, Medicaid Agency, State Employees' Insurance Board, and Public Education Employees' Health Insurance Board to collaboratively develop a plan for reducing chronic disease incidence and improving chronic care coordination.

• Plan must identify goals and benchmarks for reducing chronic disease and specify wellness and prevention programs tailored to Medicaid recipients, state/local government employees, and public education employees.

• Mandates annual reports due by January 1 to the Governor, State Health Officer, relevant House and Senate committees, and the Legislative Fiscal Office detailing goals, wellness programs, financial impact of chronic conditions, and benefits of existing prevention programs.

• Reports must include a detailed action plan for care coordination addressing hospital readmission rates, transitional care plans, medication management, quality care standards, and expected outcomes with established benchmarks.

• Becomes effective on the first day of the third month following passage and Governor approval.
