Requires all health benefit plans, contracts, and certificates executed or renewed in Alabama after the effective date to offer coverage for breast reconstruction surgery following a mastectomy.

Coverage must include all stages of reconstruction of the affected breast and surgery and reconstruction of the other breast to produce symmetrical appearance, determined by the attending physician and patient.

Applies standard deductibles and reasonable copayment or coinsurance provisions consistent with other health care services in the plan or policy.

Requires written notice of coverage availability to be delivered to policyholders upon enrollment and annually thereafter.

Prohibits insurers from denying eligibility, providing incentives to decline coverage, penalizing health care practitioners, or restricting hospital stays inconsistently based on this required coverage.
