• Allows insurance carriers to require officers revoking exemptions to execute an affidavit stating they have not suffered any employment injury since the exemption began, and prohibits revocation if the officer fails to comply.

• Establishes that exemption or reinstatement certifications become effective on the first day of the calendar month following the insurance provider's written acceptance of the certification.
