• Authorizes certified registered nurse practitioners and certified nurse midwives to sign medical forms, certifications, and endorsements that currently require a physician's signature, provided they act within their scope of practice and maintain an active collaboration agreement with a physician.

• Allows nurse practitioners and midwives to certify disability for disabled parking tags, sign physicals (bus drivers, Special Olympics, TSA/government employees, college, adoptive parents, scouts), employment absence forms, and medical equipment/supply authorizations.

• Permits signing of Medicaid forms for medication/supplement ordering and referrals, death certificates, home health recertification orders, jury duty illness excusals, ambulance transport forms, and driver's license documentation for diabetics.

• Establishes that the State Board of Medical Examiners may approve additional forms by rule after consulting with professional societies of nurse practitioners, nurse midwives, and physicians.

• Clarifies that the act does not expand the scope of practice for nurse practitioners or midwives, nor does it expand collaborating physicians' responsibilities under state law; effective immediately upon passage and approval.
