Changes the State Board of Health authority from "authorized to" to "may" create and maintain an immunization registry containing records of vaccines administered to all ages, including historical vaccination records.

Requires health care providers to review the immunization registry before administering a vaccine, unless they have a bi-directional electronic interface with the registry.

Mandates providers report all vaccine data to the registry through electronic interface or manual entry, with information obtained from clinic records, billing data, vital records, and other state-controlled sources.

Exempts annual influenza vaccinations from the registry review requirement, though providers must still report influenza vaccination data to the registry.

Takes effect immediately upon passage and approval by the Governor.
