• Establishes eligibility criteria for community paramedic services, including patients discharged from medical facilities, those identified by physicians as likely to benefit from preventive care, and those at risk of hospital admission or readmission.

• Limits community paramedic services to coordination of community services, chronic disease monitoring and education, health assessment, hospital discharge follow-up care, laboratory specimen collection, medication compliance, continuation of care within medical facilities, and integrated medical care under physician oversight.

• Prohibits community paramedics from making diagnoses or prescribing drugs, and requires documentation of all services provided with copies forwarded to treating physicians.

• Requires the State Board of Health to adopt rules implementing the program and revises definitions of emergency medical services personnel and related terms.
