Establishes a minimum reimbursement rate of 325 percent of Medicare rates for out-of-network ground ambulance services, which constitutes payment in full from health insurers.

Prohibits out-of-network ambulance providers from billing insured individuals for amounts exceeding the minimum reimbursement rate, except for the patient's in-network cost-sharing amounts (deductibles, copayments, coinsurance).

Requires health insurers to remit payment directly to ambulance providers within 30 days of receiving a clean claim, rather than paying the enrollee.

Requires health insurers to certify the enrollee's cost-sharing amount to providers upon request and prohibits enrollees from being included in payment disputes between insurers and providers.

Becomes effective October 1, 2024, and applies to health maintenance organizations and other health care insurers under specified provisions of Alabama law.
