Establishes the Community Health Worker Certification Act requiring the Alabama Department of Public Health to certify community health workers and training programs based on defined core competencies including communication, advocacy, capacity-building, and public health knowledge.

Creates a Community Health Workers Review Board cochaired by a Department of Public Health representative and a statewide association representative, with additional members from state agencies and legislative appointees, to advise on certification standards and training programs.

Sets certification fees not to exceed $200 for initial certification and $150 for renewal, with the Alabama Department of Public Health authorized to deny, suspend, or revoke certifications after notice and hearing.

Authorizes the department to collaborate with the State Department of Education, Alabama Community College System, and Commission on Higher Education to certify academic-based training programs, and with community health worker associations for community-based programs.

Clarifies that certification is optional for employment and establishes a grandfathering process for community health workers practicing before the program begins, with the act effective October 1, 2025.
