Prohibits injection, storage, or sequestration of carbon dioxide in underground wells (including EPA Class VI wells) unless approved by the governing body of the county and municipality where the well will be located

Requires applicants to pay a $50,000 application fee and submit documentation including financial information, operating experience, and any civil or criminal actions against the applicant or its affiliates

Mandates a public approval process including certified mail notice to adjacent property owners, newspaper and radio announcements, at least one public awareness session, and at least one public hearing before the governing body decides within 30 days

Imposes a 10% tax on the gross value of all carbon dioxide injected or stored, with 50% going to the state General Fund and 50% distributed to the host county (split with municipality if applicable)

Requires applicants to petition circuit court for declaratory judgment confirming the local governing body followed proper procedures before seeking other regulatory approvals; reapplication within 18 months of denial requires a $75,000 fee
