Appropriates up to $203,404,327 in federal funds from the One Big Beautiful Bill Act of 2025 (P.L. 119-21) to the Alabama Department of Economic and Community Affairs (ADECA) for the Rural Health Transformation Program for fiscal year ending September 30, 2027.

Allocates funds across 10 health initiatives, with largest amounts to Rural Workforce Initiative ($57.9 million), Rural Health Initiative ($55.9 million), and Collaborative EHR, IT, and Cybersecurity Initiative ($31.4 million).

Reserves up to $20,340,432 for administration, auditing, and reporting requirements of the program.

Requires ADECA Director to obtain written concurrence from the Finance Director and legislative committee chairs before reallocating funds between initiatives, followed by CMS approval.

Unexpended funds do not revert but carry forward; effective date is October 1, 2026.
