Health benefit plans in Alabama that cover supplemental or diagnostic breast examinations must provide that coverage without imposing deductibles, copayments, or coinsurance on enrollees

Covered examinations include contrast-enhanced mammography, diagnostic mammography, breast MRI, breast ultrasound, and molecular breast imaging

Diagnostic breast examinations evaluate abnormalities detected through screening or other means; supplemental examinations screen high-risk individuals based on personal/family history or factors like dense breast tissue

Applies to health insurers, HMOs, health care services plans, and nonprofit agricultural organizations providing health benefits; excludes limited benefit policies like accident-only, dental-only, Medicare supplement, and disability income insurance
