Appropriates up to $203,404,327 in federal funds from the One Big Beautiful Bill Act of 2025 (P.L. 119-21) to the Alabama Department of Economic and Community Affairs for the Rural Health Transformation Program for FY 2027

Allocates funds across 10 health initiatives including: Rural Workforce Initiative ($57.9M), Rural Health Initiative ($55.9M), Collaborative EHR/IT/Cybersecurity Initiative ($31.4M), Cancer Digital Regionalization ($8.1M), Mental Health Initiative ($7.3M), and Maternal and Fetal Health Initiative ($6.1M)

Reserves up to $20,340,432 for administration, auditing, and reporting requirements of the program

Requires ADECA Director to obtain written concurrence from the Finance Director and legislative committee chairs before reallocating funds between initiatives, followed by CMS approval

Unexpended funds carry forward and do not revert; effective date is October 1, 2026
