Department of Human Services must create a plan by December 31, 2012 for transitioning residents of state developmental disabilities facilities to community-based settings appropriate to their needs

Plan shall incorporate recommendations from the Governor's Integrated Services Task Force and the Olmstead Working Group report submitted February 15, 2001

Plan must include individual assessments identifying community services needed, costs, barriers to community placement, and transition timelines for each resident, with goal of moving all residents to community settings by January 1, 2018 except those requiring institutional care

Plan shall address facility property repurposing, total costs including Medicaid participation, cost savings, funding sources, legal obstacles, and required service development including crisis intervention

All new long-term admissions to state intermediate care facilities for developmental disabilities cease upon the bill's effective date, and facility expansions or construction contracts are prohibited until plan completion
