Allows persons to apply to the Office of Driver Services to obtain a living will designation on a driver's license or identification card by providing a signed form stating they have executed a living will and paying the applicable fee.

Permits duplicate or substitute driver's licenses with living will designations to use the reduced fee under § 27-16-806 if the current license has not expired.

Authorizes the Office of Driver Services to determine the placement of the living will designation on licenses and identification cards and to promulgate rules for administering this provision.

Makes living will designations on driver's licenses completely voluntary, specifying that the law does not require any person to have a living will or display a designation on their license.
