Establishes a two-year Medicaid Primary Care Case Management shared-savings pilot program beginning January 1, 2014, in 39 Arkansas Delta counties targeting 5,000 high-cost Medicaid recipients with catastrophic or chronic conditions, with savings shared 34% to the state and 66% to the vendor up to a 5% cap.
