Department of Human Services shall contract with a vendor to implement a two-year Medicaid Primary Care Case Management shared-savings pilot program in the Arkansas Delta region beginning January 1, 2014

Pilot program shall enroll minimum of 5,000 recipients with catastrophic/chronic conditions or high-risk pregnancy history, whose estimated costs are in the top quartile, and who are not enrolled in similar programs

Program covers 39 specified Arkansas Delta counties and excludes members of disability waivers and Programs of All-Inclusive Care for the Elderly

Savings shall be shared 34% to the department and 66% to the vendor (capped at 5% of total costs), with 50% of vendor savings shared with participating primary care providers based on performance standards

Physician participation is voluntary with no penalty for refusal or contract termination; savings calculated after 15 months of operation using first 12 months of data plus 3 months run-out, then quarterly thereafter
