Department of Correction must collect demographic and location data for all incarcerated persons, including a unique identifier, facility address as of April 1 each year, pre-incarceration residential address, race/ethnicity, and age.

Department shall deliver an annual report to the Governor by May 1 containing collected data and percentages of incarcerated persons by race and ethnicity.

Reports must include total number of persons in custody with racial and ethnic breakdowns for known data.

Collected data is confidential and cannot include individual names or identify specific persons; disclosure is restricted to aggregated data by census block, tract, or zip code.
