Creates the Arkansas Works Program, a health insurance program for eligible individuals aged 19-64 with income at or below 138% of federal poverty level, to be administered by the Department of Human Services in coordination with the State Insurance Department.

Provides health insurance through three methods: individual premium assistance for enrollment in silver-level qualified health plans through the marketplace, employer-sponsored premium assistance for eligible individuals, and supplemental benefits to incentivize personal responsibility.

Requires program participants earning at least 100% of federal poverty level to pay premiums of no more than 2% of income; establishes wellness visit requirements and work/job training participation incentives for eligible individuals.

Mandates program termination within 120 days if federal medical assistance percentages fall below specified thresholds (95% in 2017, 94% in 2018, 93% in 2019, 90% in 2020 or later), with the subchapter expiring December 31, 2021.

Terminates the Health Care Independence Program on December 31, 2016, and creates the Arkansas Works Program Trust Fund to receive premium tax revenues and other savings to fund future program obligations.
