Creates the Patient Right-to-Know Act in Arkansas Code Title 20, Chapter 6, requiring entities that employ healthcare providers to provide patients and providers with contact information when a provider relocates their practice.

Prohibits entities from misleading patients about a healthcare provider's new practice location or contact information, or failing to provide such information when requested.

Requires entities to provide relocating healthcare providers with a list of existing patient names and addresses within 21 calendar days, and to send notices or post website information about the provider's new location within the same timeframe.

Establishes that violations constitute unfair and deceptive practices under the Deceptive Trade Practices Act, allowing patients or healthcare providers to seek injunctions and recover liquidated damages of $1,000 per day per violation or actual damages, plus attorney's fees.

Declares an emergency to take effect immediately upon gubernatorial approval to ensure continuity of patient care and prevent disruption of healthcare provider-patient relationships.
