Healthcare insurers must decide on physician and physician assistant participation applications within 60 calendar days of submission, and other provider applications within 180 calendar days.

Insurers must provide written acknowledgement within 10 days of receiving an application and notify applicants within 15 days if the application is incomplete, with a list of required items.

Insurers must notify physicians, physician assistants, and advanced practice registered nurses at least 90 days before recredentialing deadlines and at least 45 days before termination for failure to recredential.

Providers who change employment or location but are already credentialed require only additional information necessary for the change, not a new application.

Insurers face a fine of $1,000 per day for each day exceeding the 60-day decision deadline for physicians and physician assistants.
