Requires physicians at abortion facilities to electronically report abortion complications to the Department of Health within 3 business days; healthcare facilities must report within 30 days.

Defines "abortion complication" to include shock, uterine perforation, cervical laceration, hemorrhage, infection, sepsis, death, incomplete abortion, damage to the uterus, and infants born alive after abortion procedures.

Reports must include specific information such as abortion date, type, gestational age, patient demographics (year of birth, race, marital status, state/county of residence), and medical history, but cannot identify the physician or patient.

Department of Health must publish an annual aggregated statewide report on the department's website without duplicative data.

Violations subject violators to $500 civil penalty per violation; third violation may result in license revocation or suspension; Attorney General notified of physician violations.
