Prohibits covered entities (healthcare providers, hospitals, transplant centers, and organ matching entities) from denying organ transplants or related services based solely on an individual's mental or physical disability

Requires covered entities to make reasonable modifications to policies and procedures, including communicating with support networks and considering available support systems when evaluating post-transplant compliance ability

Mandates provision of auxiliary aids and services such as qualified interpreters, accessible information formats, supported decision-making assistance, and assistive technology to qualified individuals with disabilities

Allows disability to be considered in transplant decisions only when a physician conducts individualized evaluation and finds the disability medically significant, and permits referral to specialists and placement on waiting lists without disability-based discrimination

Establishes legal remedies allowing qualified individuals to bring circuit court actions for injunctive or equitable relief, with expedited hearing requirements, while preserving remedies under the Americans with Disabilities Act
