Allows a parent to appoint a temporary guardian for a minor child by signing a notarized statement that includes the child's name and date of birth and the appointed guardian's name.

Requires two witnesses to sign a sworn attestation confirming they witnessed the parent sign the notarized appointment statement.

Requires both parents to jointly sign and notarize the appointment if both parents are alive, and the appointed guardian must consent in a signed and notarized statement.

Temporary guardianship becomes effective immediately upon the death of the appointing parent (if the other parent is deceased) or upon the death of both parents, lasting no more than 45 days with possible court extension of up to 90 additional days.

Courts must comply with the parent's wishes and name the appointed temporary guardian as permanent guardian unless the person is determined to be unfit.
