Creates the Alzheimer's Disease and Dementia Advisory Council consisting of 20 members including two legislative cochairs, representatives from healthcare organizations, Alzheimer's advocacy groups, state agencies, and individuals with relevant expertise in dementia care and research.

Council members serve two-year terms with voting members appointed by various state officials and nonvoting representatives from the Department of Health, Department of Human Services, and the long-term care ombudsman.

Council shall meet at least quarterly and examine needs of individuals with Alzheimer's disease and dementia, available services, and healthcare provider capacity to develop a comprehensive "State Alzheimer's Plan" with findings and recommendations.

Requires council to submit annual reports by October 1 each year starting in 2022 to the Governor and legislature on implementation status of plan recommendations, barriers, and proposed legislation, with updated plans issued every four years.

State agencies identified in the plan must submit biennial implementation reports by October 1 of even-numbered years detailing steps taken and reasons for any failures to implement recommendations.
