Department of Health shall send all abortion data collected under state law to the Centers for Disease Control and Prevention and make this data publicly available through the Division of Vital Records.

Notifying an abortion facility of a planned inspection date or time is a Class A misdemeanor; knowingly concealing or removing inspection-related information is also a Class A misdemeanor.

All abortion facility inspections must be conducted by at least two persons simultaneously.

Physicians and abortion facilities must report to the Department of Health the number of abortions performed to save the life of the mother and the number performed due to rape or incest.

Abortions for rape or incest pregnancies require documentation that the crime was reported to law enforcement; such documentation must be filed in the pregnant woman's medical record and the procedure must be reported to the Department of Health.
