Defines "prosthetic device for athletics or recreation" as a device enabling high-impact activity exceeding basic ambulation skills, including blade-type feet designed for running.

Limits replacements to once every three years unless medically necessary, and allows plans to require prior authorization and impose copayments comparable to other covered benefits.

Requires recreational prostheses to be prescribed by a licensed physician and provided by a qualified medical professional, with medical necessity determined by the treating or referring physician.

Establishes that candidates for recreational prostheses must qualify as Medicare functional level K-3 or K-4 users capable of high-level activity pursuits and performance beyond normal ambulation.
