Absentee ballot applications must be delivered blank except for information automatically populated by the voter registration system, with no pre-filled voter selections allowed.

Applications must be signed by the applicant and verified by the county clerk using voter registration information, or if sent electronically, must bear a verifiable facsimile of the applicant's signature.

Any person providing assistance to an applicant filling out an absentee ballot application must provide their name, address, and signature on the application, and this information must be included on the application form.

County clerks may only deliver absentee ballots to persons other than the voter if they are a designated bearer, authorized agent, or facility administrator, and they must provide current and valid federal or state-issued photo identification.

A person who purposely violates restrictions on absentee ballot delivery is guilty of a Class A misdemeanor upon conviction.
