Transfers administration of the Life Choices Lifeline Program from the Department of Health to the Department of Human Services and clarifies program components including services for pregnancy assessment, medical information, obstetric care, and mental health counseling.

Creates the Continuum of Care Program within the Department of Human Services to provide statewide telemedicine support, community outreach, and care coordination for pregnant women and parents, with services targeted to biological parents of unborn children and adoptive or biological parents of children under two years old.

Prohibits the Department of Human Services from contracting with abortion providers or organizations that assist with, refer for, promote, or employ individuals who have performed abortions in the past two years, and protects conscience objections by contractors.

Transfers resource access assistance functions under the Every Mom Matters Act from the Department of Health to the Department of Human Services, including establishing a toll-free number, secure database system, and website for pregnant women seeking abortion to receive information about available services.

Requires contracted organizations to report monthly on service metrics including number of individuals served, types of referrals provided, and support needs identified, while maintaining confidentiality of participant information.
