Creates the Continuum of Care Program within the Department of Human Services to provide telemedicine support, community outreach, and care coordination for pregnant women and parents, with services funded by General Assembly appropriations.

Establishes that the Department of Human Services (instead of Department of Health) administers the Life Choices Lifeline Program and related services, including care plan coordination and healthy pregnancy program services for pregnant individuals.

Prohibits contracting with organizations that are abortion providers, assist women in obtaining abortions, employ individuals who performed abortions in the last two years, or have board members/officers/employees meeting these criteria.

Transfers administration of resource access assistance offers under the Every Mom Matters Act from the Department of Health to the Department of Human Services, requiring a toll-free hotline and secure database to connect pregnant women seeking abortions with agencies offering alternative services.

Requires contractors to report monthly on services provided, including number of participants served, types of referrals made, and support needs identified; prohibits requiring referrals to services to which agencies have conscience objections.
