Screening mammograms cannot be subject to copayments, deductibles, or other cost-sharing requirements under health benefit plans, regardless of whether services are provided on the same or different dates

Breast ultrasounds are exempt from all cost-sharing requirements, including when multiple services occur on the same date or across different dates

Cost-sharing for diagnostic breast cancer examinations, including breast MRI, must be no less favorable than cost-sharing for screening examinations

Mammography services must be performed at credentialed or accredited facilities to qualify for insurance payment
