Expands the definition of "healthcare insurer" to include entities that administer health insurance (not just provide it), and adds third-party administrators acting on behalf of self-insured health benefit plans to the list of covered entities

Prohibits contracting entities from enforcing any stipulation, requirement, or policy that has the effect of an all-products clause, even if such a clause is not explicitly included in the healthcare contract

Broadens the prohibition on leasing, renting, or selling healthcare contracts or provider networks to apply to "contracting entities" rather than just "healthcare insurers"

Requires contracting entities that lease provider networks to provide notice to healthcare providers and allow them to opt out of participating in the lease arrangement, with an exception for dental-only plans and transfers between affiliates or same brand licensees

Prohibits any waiver of the subchapter's requirements by contract, rendering void any contractual arrangement or action that conflicts with or purports to waive the law's provisions
