Creates a petition-driven election process allowing registered voters served by public water systems (serving 5,000+ persons) to vote on whether to permit or prohibit water fluoridation

Requires petitions to be signed by 38% of registered voters served by the public water system and filed at least 120 days before the election date

Elections held during general/primary elections in even-numbered years or on the second Tuesday of March or November in odd-numbered years, with election costs paid by the public water system's board of directors

If a majority votes to prohibit fluoridation, the water system must cease fluoridation within 30 days of certification; subsequent elections on the same issue cannot be held for at least 4 years

Establishes detailed requirements for petition canvassers including photo ID verification of signers, notarized affidavits, and criminal penalties (Class A misdemeanor or Class D felony) for petition fraud, forgery, or false statements
