Amends Arizona Revised Statutes Section 28-669 regarding accident report forms prepared and supplied by the Department of Transportation to police departments, sheriffs, and other agencies.

Clarifies that written reports required to be made by investigating officers must call for sufficiently detailed information about traffic accidents including cause, existing conditions, and persons and vehicles involved.

Requires accident reports be made on the department-approved form and contain all required information unless it is not available.

Authorizes the department to modify the written accident report form in conjunction with state and local law enforcement agencies, engineering agencies, and emergency medical service organizations.
